Appointments

Our office accepts new patients every day! We appreciate the opportunity to provide medical care for the whole family.

We see patients on an appointment basis, and ask that you call in advance so that we can reserve a specific time for you. Please arrive 15 minutes prior to your appointment to update any and all insurance and/or personal information. To help with this process, please click on our tab and print out the New Patient Forms, Patient Update Form, Patient Health History, and bring that with you to your appointment. Please bring your insurance card, photo ID, medication list.

For patients under the age of 18 and needing to be seen without a parent/guardian present, we will need a permission signed by the parent/guardian prior to the patient being seen - Minor Treatment Authorization.

Insurance

We accept most major insurance plans including Medicare and Tricare. We also provide services for several of the large HMO plans in our area, and participate in many Preferred Provider Organizations.

Payment Information

One of our goals at Colorado Springs Family Practice is to provide high quality, cost effective medical care. Our fees are based on quality of materials we use, as well as our training and experience in performing your needed treatment.

We understand that while having insurance, there may be out of pocket expenses, deductibles and co pays; our office will try to accommodate any situation and make remaining balance affordable.

We accept payments by check, cash, debit and credit cards.

Prescriptions & Refills

Prescription refills may be requested by calling your pharmacy or our office. Be sure to call at least one week before you run out of medication.

For your safety, all medications taken on a regular basis for a chronic illness require regular follow-up visits before they can be refilled.

Office visits are required for prescription and refill requests for antibiotics, pain medications, and other controlled substances. On-call providers cannot order refills for these medications after regular office hours.

We are happy to provide our patients with medication samples when available and appropriate. However, we will only be able to dispense samples of medication at an office visit. We are also limited in the amounts that we can give in order to be able to provide samples for as many patients as possible.

Diagnostic/Test Results

Results of most Laboratory or other Diagnostic Tests will be available to you within ten days of the test date. In most cases, we will notify you of test results within that time or in cases requiring urgent notification, we will notify you sooner. If you have not received notification of results within 14 days, call us at (719) 634-8891.

Test results for lab work, X-rays, etc. are sent directly from the processing or service provider to the patient, usually within 10 days. If you have not received a report after 10 days, please notify our office.

Emergency Care

  • In the event of a life-threatening emergency - call 911 or proceed to the nearest emergency room.
  • If you are not sure about the seriousness of a medical situation and feel that any delay is unacceptable, then proceed to the nearest emergency room.
  • If the situation is not severe, but requires attention while our office is closed, please call our office at 719-634-8891 and you will be directed to our on-call provider.
  • If you have a concern or question while our office is open, call the office and the provider will be in touch with you directly or through office personnel. If your provider feels it necessary, you may be asked to come in for a work-in appointment.

Hospitalization

To provide the best medical care in the event you require inpatient services, there will be a group of physicians who are hospitalists and focus strictly on inpatient or “in hospital” medicine. These are excellent physicians who work closely with Colorado Springs Family Practice to carefully coordinate care.
