Office Policies & Procedures

Important Patient Office Policy Information
In order to better serve our patients and make each visit as efficient as possible, we ask all patients to make note of the following prior to their scheduled visit:

All Co-Pays are due at the time of service.

Please arrive at least 15 minutes before your scheduled visit to update any and all insurance and/or personal information.

Test Results:
Test results for lab work, X-rays, etc. are sent directly from the processing or service provider to the patient, usually within 10 days. If you have not received a report after 10 days, please notify our office.

Medications:
For your safety, all medications taken on a regular basis for a chronic illness require regular follow-up visits before they can be refilled.

Office visits are required for prescription and refill requests for antibiotics, pain medications, and other controlled substances. On-call providers cannot order refills for these medications after regular office hours.

We are happy to provide our patients with medication samples when available and appropriate. However, we will only be able to dispense samples of medication at an office visit either with the physician, nurse practitioner, physician assistant, or office nurse. We are also limited in the amounts that we can give in order to be able to provide samples for as many patients as possible.

Work-In Appointments:
To provide patients more scheduling options, physicians, nurse practitioners, and physician assistants are available to render our quality medical services. Urgent or same-day appointment requests may be made with any provider with available time slots.

Administrative Functions:
An authorization for Workman’s Compensation coverage will be required for all work-related visits.

Because many insurance plans limit same-day services, we do not perform procedures on the same day as non-surgical office visits.

We are asked frequently to prepare letters, sign forms, rewrite prescriptions due to changes in insurance or pharmacy benefit plans, provide medical documentation for FMLA, disability qualifications, etc. We are happy to assist you in these requests, but we will ask patients/responsible parties to assume financial responsibility within the scope of an office nurse visit for prescription related actions (this may be billed to your insurance company and a co-pay may apply), or prepayment for forms and medical summaries not completed during an office visit.


Remember to bring all medications you are currently taking, in prescribed containers, to each visit.



We cannot use a patient’s scheduled office visit to treat or examine accompanying family member(s).



We ask that you please refrain from using cellular telephones within the office premises. These may interfere with some of our equipment.
